Medication History does not always display drug information for one or more of the following reasons:

• Some pharmacies wait until the patient picks up their medication before recording it.
• Downstream timeouts can occur.
• The patient withdrew consent for their drug information to be shared.
• The medical service plan and/or pharmacy do not provide data.

Medication History will typically show if a patient is covered and reported as dispensed to the provincial Drug Information System(Digital Health Drug Repository known as the DHDR in Ontario and PharmaNet in BC).This is dependent on each provincial jurisdiction and if they are actively reporting it to the Provincial Drug Information System. 

• Dispensed medication history comes from the pharmacies only.
