- First Name
- Last Name
- Date of Birth
- Postal Code
- Zip Code
- Provincial health number (PHN) (unique patient identifier assigned by the provincial medical plan)
- If issue relates to SmartSig, what is the specific medication, filled date, and dispensed information (i.e., dosing including: strength and unit of measure, days supply, quantity sig, if available)
- Date & Time Stamp
- Error messages (if applicable)
