MedHx: Why Can’t I See Patient Information in Medication History?

  • If you can't see patient information in their medication history, the patient information likely does not match what their provincial service plan has on file (e.g., exact name match).
  • Only publicly funded medications, narcotics and controlled substances are stored within the provincial drug information system which is the source for medication history.  
  • As the Ministry of Health adds other non-publicly funded medications to the Drug Information System then they will become available.

Certain information may not be available or accurate in this report, including:

  • Items that the patient asked not to be disclosed due to privacy concerns,
  • Over-the-counter medications,
  • Low-cost prescriptions,
  • Prescriptions paid for by the patient, or
  • Non-participating sources or errors in insurance claims information
  • Information not recorded by the Provincial Drug Information System 

The provider should independently verify medication history with the patient.
